
CAN'T GET A CHANGE FORM

FOR TWO YEARS NOW I HAVE TRIED TO GET A CHANGE FORM FROM SECURITY BLUE A HIGHMARK HMO IN WESTERN PA. I WANT TO CHANGE FROM THEIR EXPENSIVE DELUXE PLAN TO THEIR LESS EXPENSIVE STANDARD. IN 2005 I COULD NEVER GET THROUGH ON THE TELEPHONE TO GET THEM TO SEND THE FORM. THEN IT WAS TOO LATE. THIS YEAR, 2006 FIRST THEY SAID THEY DIDN'T HAVE THE FORM YET (IN OCTOBER). THEY DID NOT INCLUDE IT IN THE PACKET AND IT CAN'T BE DONE ANY WAY BUT ON THE FORM. THEN THEY SAID YES THEY WOULD SEND IT TO ME BUT IT NEVER CAME(2 WEEKS AGO). DO I HAVE TO KEEP PAYING FOR THE DELUXE PLAN THAT I DO NOT WANT FOR ANOTHER YEAR??? WILL I NEVER GET THE FORM AND NEVER BE ABLE TO GET THROUGH AGAIN AS THE TIME GETS SHORT??? I WILL BE LEAVING AT THE END OF NOVEMBER. HOW CAN I GET THIS SECURITY BLUE PLAN CHANGED??? I AM VERY FRUSTRATED. THIS IS BECOMING EXTREMELY REDICULOUS!!!!
