Nearly half have waited more than six months.However, doctors blamed the rise on the squeeze in budgets that is increasingly being seen in the health service. Peter Kay, president of the British Orthopaedic Association and a former adviser to the Department of Health, said the sheer numbers waiting longer than 18 weeks meant it was unlikely to be solely for personal or medical reasons. He said the focus of hospitals was now slipping and patients were being left waiting for too long. And he added: 'One of the issues with 18 weeks is that once you have missed the boat there is less incentive to get the patient treated. They get left on the list for longer than they otherwise would.'

But her appointments to go under the knife were cancelled four times and she barely regained consciousness after finally having surgery. Her devastated husband, Jim, is now demanding answers from Mid Essex Hospital Services NHS Trust--the organisation where his wife had served as a non-executive member of the board of directors. He said: 'I don't really know why she died. I did not get a reason from the hospital. We all want to know for closure. She got weaker and weaker as she waited and operations were put off.' It would be cruel to put this down to karma, so instead we'll just note that it can't possibly be true. After all, 'In Britain, the government itself runs the hospitals and employs the doctors. We've all heard scare stories about how that works in practice.'

The urban landscape of Hanley, a place to which I have returned is a living demonstration of the failure of an economic model. There are more than enough pawnbrokers and cheque cashing stores, some temp agencies, with very meagre offers in the window. And at night, silence - save for the trashy student bars where, say the posters, you can get 'trollied' for 99p. Something about the economic model we adopted has just not worked.
