not understanding

why is it that when you call a business, for example, a doctors office, you can not get someone whom you can understand. is it too hard to hire someone who speaks english clearly. when i call a doctors office to get a prescription refilled, i would like know that the person who i am speaking with can understand me. any responses are welcome
