My wife's step-mom and I hooked up for a while back before any of us knew each other. As far as I know, she hasn't told her husband, either. She and I are totally friends now but I can tell she still thinks about it.

Do everything you can to only be around her at Family functions, etc. And tell no one. This can serve absolutely no useful purpose other than inflict pain. You have a wife now and your complete devotion should be to her. They call it the "Past" for a reason. And if you have to, remind your wife's step-mom of this should something happen. But don't allow yourself to be alone with her, and nothing should.
