Im scared of my toilet seat

I haven't gone to use my toilet in years. I don't know why but my toilet scares me so much I have to shit outside, in a hole. It's even hard to go into the bathroom, I had a panic attack a while ago because I almost fell in the toilet when I slipped on my moms underwear. I screamed for an hour because no one was home and I passed out.
