Dr. Mac Psychiatry 29525 Canwood St. Agoura Hills, Ca 91301. i feel he is a very lazy Dr. He doesn't come through with what he should should be doing he wont call in preauthorzation request to insurance CO. It has been 2+months and he still has not helped me at all.
