I want to come out to my wife but I am so very, very scared to tell her I am gay. It frightens me to the point of a panic attack and I have thought about just running. We don’t have any kids, so there is that at least.

Ask her if she wants to have a 2nd man join you to plug her holes. Make it about satisfying her. You don't have to tell her (at least at first) that you really want the other man for yourself. I would suggest telling her. Living a lie is not healthy for your relationship or for you own good. Will there be some reaction from her? Definately. And it might be hard for her to accept it in the beginning, but you can't change the fact that you are gay. You would also do her a favor telling it. That way she can find somebody who actually loves her (sorry if that sounds harsh, but it's the truth) I hope this helps :)
